HEALTH INSURANCE
New full time students with at least 9 hours graduate or 12
hours undergraduate, and all international students entering
FSU must show
proof of
health insurance
at Florida State University.
Students
not cleared for this requirement at the beginning of the Drop/Add
period will be blocked from registration. For more information
call (850) 644-6573.
Click on the button below to go through our online tutorial
on health requirements and to show proof of adequate health insurance
or
to purchase
the university-sponsored health
insurance. DO NOT FAX OR MAIL YOUR INSURANCE CARD. YOUR WAIVER
MUST BE COMPLETED ON LINE.
Have you waived the insurance requirement before? Are you ready
to renew or review your information? Do you wish to renew your
school sponsored insurance now? If so, click on the “Returning Student” button below.
It will take you directly to the log in screen. Once you log
in, select either the ‘Insurance Waiver’ button or
the ‘Purchase FSU Insurance’ button to complete your
transaction.
TSHC Insurance Information
For information
regarding the university-sponsored insurance plan, please
go to www.aetnastudenthealth.com or
call the Thagard Insurance Office at (850) 644-4250. This
phone number will direct you to the insurance broker for
FSU. If they are not able to answer your questions they
will refer you back to FSU’s Health Compliance office.
You must show proof of health insurance at the beginning
of each academic year.
It does not matter what insurance you have. Your student
can always be seen at Thagard Student Health Center.
If
you have the school sponsored health insurance and you still
have not received
your
permanent health insurance card, check your local mailing address
on file at your blackboard account. That is the address to
which the card will be mailed. If you have not updated your
address,
your permanent card will have a tough time reaching you.
Temporary cards for school-sponsored insurance: If it has
been at least two weeks since you purhased your plan and the
school term has begun, log into the screen where you purchased
your plan. If a policy number has been assigned to you, you
will see a button to print a temporary card.
We are invested in your academic
success. Any illness, accident or injury could
deter you from your goals unless you can access affordable diagnostic
testing, treatment and follow-up care. Health insurance is one
tool to make your health care affordable.
A health insurance plan must be comparable to the university-sponsored
health insurance plan. For students without adequate health insurance
coverage, FSU offers an affordable health
insurance policy. Students not cleared
for the insurance
requirement at the beginning of the registration window will
be blocked from registration. This does not mean students will
have to buy the university-sponsored insurance, but if they choose
not to, they must show that they have current, valid health insurance
with another carrier that provides similar coverage to
that of the university-sponsored policy:
Another plan must meet simple requirements:
1. Can you access physician and hospital care in this geographical
part of Florida?
2. Does your plan have in patient and out patient mental health
coverage?
3. Does your plan have prescription coverage?
4. Does your plan cover the student 365 days a
year?
5. Does it have a benefit of $250,000 annual coverage?
6. Does
your plan cover maternity care (for female students)?
4.
Will my current insurance cover costs at Thagard Student
Health Center?
The pharmacy accepts more than 100 prescription insurance plans.
Click
here to
see a list of currently accepted plans. We
only accept the university-sponsored insurance and Aetna insurance
as payment for medical services (for lab, x-rays, or other
procedures, we will help you file claims). Make
the request at the cashier as you check out at the end of your
visit. Have your insurance card
with you.
Your insurance company will review the charges
and determine what they will reimburse you. This process
usually
takes four to six weeks and is always subject to your deductible
and coinsurance. Tthe insurance billing office
staff cannot
tell you the amount or an exact date of your reimbursement.
Filing a claim does not mean you do not have to pay your
bill to TSHC,
nor is there a guarantee that you will receive any money
back from your insurance company. Deferred charges are sent
to Student Financial Services daily and remain on
a student account until they are paid. Any on line activity, such
as registration or seeing your grades, stops as long as you have
an amount due on your student account.
*If you are with Tricare Prime, you may not receive reimbursement
for services rendered in the clinics at Thagard Student Health
Center unless you are first referred by your Primary Care Manager.
Our pharmacy can accept Tricare Prime for the purchase of prescriptions.
Tricare Standard enrollees are not affected by this requirement.
5.
How do I get a prescription with an insurance card at Thagard's
pharmacy?
Bring your insurance card with you. Once
a prescription has been written, you need to present your insurance
card to the pharmacist at the SAME time the prescription is turned
in. Once you have presented your insurance card to the pharmacy,
you will not have to present the card again as long as the insurance
plan does not change or expire. The pharmacy keeps the insurance
information on file for you. Co-pays do apply.
8.
I am not a student. May I purchase the university-sponsored
health insurance?
Post doctoral fellows and cisiting scholars may purchase the
university-sponsored health insurance and use the services at
the health center.
International students covered by a medical plan that does not
offer evacuation and repatriation coverage may purchase that
coverage from Aetna Student Health.
All of these applications are available in PDF format. Print
the application, fill it out, and take it to the Health Compliance
Office (room 135) for processing. Be prepared to provide credit
card (MasterCard or VISA) or debit card (with MasterCard or VISA
logo) information or a check made out to Aetna Student Health
for the full amount of insurance being purchased.
9.
I need needlestick coverage. Is that available?
Aetna Student Health does provide a needlestick
plan. The application is available on this website in PDF format.
Print the form, complete it, and forward the application with
payment for the coverage to the address at the end of the application.
10.
What is the percentage of coverage provided by the university-sponsored
health insurance plan?
For full details please click on the brochures below:
12.
Do you keep a copy of our insurance card on file?
No, we do not keep a copy of the insurance card. The pharmacy
can put the student’s insurance information into the pharmacy's
computer system. We do maintain some insurance information in
our system for filing of insurance claims. For all
other areas and procedures within the health center, the student
will
need to carry a copy of the card. The student should always have
a copy of the insurance card for emergencies and notify the health
center, including pharmacy, when there is any change in their
coverage.
13.
I am graduating in December. If I purchase the university-sponsored
health insurance plan will I still be covered after I graduate?
Graduating students may purchase fall only or annual coverage
for that extra bit of protection during their transition from
FSU to other pursuits. If the student purchases the fall semester
only, coverage will continue to the last day before the beginning
of spring semester. Annual coverage will cover the student through
the last day before the beginning of the following fall semester.
Aetna Student Health does not provide continuation coverage for
students
who
no longer meet eligibility requirements but who have been insured
with them previously with no break in coverage. They will gladly
assist the graduating student transition into another Aetna plan
that meets the graduate’s needs. Go to www.aetnastudenthealth.com and
click on"Students." At the top of the Student Connections page,
click on "Graduating?" for more information.
14.
I have looked everywhere for how to take advantage of the graduate
subsidy. Please help me!
GA's, RA's, TA's, University and Presidential fellows wishing
to take advantage of the graduate subsidy to pay for all or part
of their insurance premium must purchase
the University-sponsored health insurance
plan ONLINE.
At the
end of drop/add the appropriate subsidy will be applied to your
student account through financial aid. Awards will be
made according to the graduate studies guidelines. If you have
any questions, please feel free
to
contact
Brian Jones
at
644-3501 or
bkjones@fsu.edu.
15.
I am going out of town for an extended time and will need
to stock up on my medications. If I have the school sponsored
insurance
plan, what do I need to do?
If you have the school sponsored insurance and you want to request
a prescription limit override, you must call Aetna Pharmacy Management
at 1-800-238-6279 for that override. Do not come to the health
center to request a prescription override. Aetna
Pharmacy Management
has advised that the insured student must request any override.
16. What is Thagard’s
insurance cancellation policy?
Individuals who purchase the school sponsored insurance for
the fall, spring/summer or summer terms may only request cancellation
of the policy before the end of open enrollment. Requests for
cancellation before the end of open enrollment for any term may
be submitted directly to the Compliance Office at Thagard. Before
the cancellation will be approved or processed for students required
to show proof of comparable coverage,, proof of comparable insurance
must be provided to the Compliance Office.
Requests
for cancellation after the end of open enrollment will not be
considered unless the insured is entering the military. See the
plan brochure for details.
