Before
you enroll at FSU, you must confirm that you have received the
following required immunizations:
- 2 MMR's (Measles, Mumps, Rubella)
- Meningococcal Meningitis*
- Hepatitis B*
*Meningitis and
Hepatitis B vaccines are strongly recommended, but at the present
time
they may be declined by signing a waiver in Section B
on the health
history form (pdf) after you review the information regarding
the diseases. The meningitis vaccine is especially recommended
for those students who will be living in residence halls or living
in close quarters (Greek housing, apartment complexes, etc.).
It is safe to receive and we do have the vaccine at the health
center. We also offer the Hepatitis B series and can begin or
complete the series. Call (850) 644-4567
for an
appointment.
Please note: In the recent past,
there has been a large mumps outbreak affecting mainly college-age
students that has spread to at least 11 states. Complications of
mumps can be severe, leading to hospitalization and in rare cases,
death. Following the recommendations of the Centers for Disease
Control and Prevention (CDC) and the American College Health Association
(ACHA), Florida State University now requires documentation of
receipt of two doses of MMR (Measles, Mumps, and Rubella) or acceptable
evidence of immunity. This supersedes the previous requirement
of two measles and one rubella shot.
For more
information on immunization requirements, call (850) 644-6573.
